Integrated Care Management

The Integrated Care Management program is here to help members understand their health and get them the support they need. Our case management team can help members:

  • Make appointments for both physical and behavioral health providers
  • Review and provide education to follow medication prescriptions
  • Connect with community resources
  • Track their vital signs through the Remote Patient Monitoring tablet (when appropriate)
  • Obtain needed supplies and equipment
  • With resources and education about their illnesses and connect them to state health programs
  • Other care options such as Home Health Care, Palliative Care, or Hospice Care through education and evaluation
