Integrated Care Management
The Integrated Care Management program is here to help members understand their health and get them the support they need. Our case management team can help members:
- Make appointments for both physical and behavioral health providers
- Review and provide education to follow medication prescriptions
- Connect with community resources
- Track their vital signs through the Remote Patient Monitoring tablet (when appropriate)
- Obtain needed supplies and equipment
- With resources and education about their illnesses and connect them to state health programs
- Other care options such as Home Health Care, Palliative Care, or Hospice Care through education and evaluation
