The Integrated Care Management program is designed to assist members with locating and making appointments to medical providers,  review and education of medications, tracking vital signs, and providing diabetic supplies and other resources. Members may also be eligible for Home Health Care, Palliative Care, or Hospice Care services through the Integrated Care Management program.
